Ghana’s Free Primary Healthcare Initiative Surpasses 26 Million Enrollments as NHIA Pushes for Universal Health Coverage

The National Health Insurance Authority (NHIA) has made a significant milestone in Ghana’s healthcare landscape, announcing that over 26 million Ghanaians have now been enrolled under the National Health Insurance Scheme (NHIS) since the rollout of the Free Primary Healthcare (FPHC) policy. This achievement represents a national coverage rate of 76 percent, marking a substantial stride toward the government’s broader goal of ensuring universal access to healthcare services across the country.

Speaking at the Upper East Regional launch of the FPHC initiative, Mr. Sebastian Alagpulinsa, the Director of Membership and Operations at NHIA, delivered a statement on behalf of Dr. Victor Asare Bampoe, the Authority’s Chief Executive Officer. Alagpulinsa emphasized the NHIA’s unwavering commitment to expanding insurance coverage and sustainable healthcare financing, describing the FPHC policy as “a major breakthrough in healthcare delivery in Ghana.”

A Strategic Alignment with Universal Health Coverage (UHC) Goals

The FPHC initiative is a cornerstone of Ghana’s push toward Universal Health Coverage (UHC), a global health objective that prioritizes access to quality healthcare, widespread population coverage, and financial protection for all citizens. Alagpulinsa highlighted that the NHIA’s role extends beyond mere financing—it is also tasked with ensuring mass enrollment of uninsured Ghanaians to guarantee seamless healthcare access.

“From our perspective, we are not just financing healthcare; we are ensuring that every Ghanaian is registered under the insurance scheme so they can access the care they need,” he stated. “This is a critical step in achieving our UHC vision.”

Record Enrollment and the “Storm” Initiative

The NHIA’s recent enrollment figures reflect the success of a nationwide mass registration campaign dubbed the “Storm” initiative, launched to accelerate FPHC implementation. Alagpulinsa proudly declared that the Authority has “covered over 26 million people—representing 76 percent of the population—a remarkable achievement.”

With the government’s target set at 80 percent population coverage for UHC by the end of the year, Alagpulinsa expressed confidence in meeting this deadline. “There is every indication that by the fourth quarter, we will surpass the UHC coverage benchmark,” he asserted, underscoring the NHIA’s determination to consolidate healthcare access nationwide.

Early Disease Detection and Long-Term Financial Sustainability

Beyond immediate enrollment benefits, the FPHC policy is positioned as a long-term investment in public health. Alagpulinsa stressed that early detection and treatment of diseases—particularly those that could otherwise lead to costly complications—would reduce the financial burden on the NHIS in the future.

“We are particularly focused on early intervention for conditions that, if left untreated, could lead to significant expenses for the NHIS,” he explained. “By addressing health issues at their source, we not only improve patient outcomes but also ensure the sustainability of our healthcare system.”

Addressing Challenges: Tariffs, Claims, and Operational Efficiency

Despite the progress, the NHIA acknowledges ongoing challenges within the NHIS, including concerns over healthcare tariffs and out-of-pocket payments by some insured individuals at healthcare facilities. To mitigate these issues, the Authority has reviewed tariff structures and is implementing a new initiative called OTAC (Online Tariff and Claims System) to enhance transparency and credibility in claims processing.

“We have taken note of the tariff-related concerns and are actively working to refine our systems,” Alagpulinsa revealed. “The OTAC initiative will ensure that claims data is more accurate, helping to sustain the financial health of the NHIS while maintaining service quality.”

Strengthening Partnerships for Policy Success

The NHIA’s success under the FPHC policy is not solely dependent on internal efforts but also on collaboration with multiple stakeholders. Alagpulinsa emphasized the importance of engaging development partners, healthcare providers, political leaders, traditional authorities, and communities to ensure the policy’s sustained success.

“We are fully committed to this initiative and ready to strengthen our partnerships with all relevant entities,” he stated. “From traditional leaders to healthcare professionals, everyone must play their part to make the Free Primary Healthcare Policy a lasting success.”

A Vision for Equitable Healthcare Under President Mahama’s Leadership

The FPHC initiative aligns with the broader healthcare reforms championed by President John Dramani Mahama, who has consistently prioritized equitable access to healthcare services as a cornerstone of national development. Alagpulinsa’s remarks reflect the NHIA’s optimism that the policy will not only improve healthcare delivery but also contribute significantly to Ghana’s UHC aspirations.

As the NHIA continues its enrollment drive and system improvements, the 26 million milestone serves as a testament to the policy’s growing impact—proving that universal healthcare is within reach for millions of Ghanaians. With sustained efforts, the nation is one step closer to a future where quality healthcare is accessible to all.
